Providing support and Services after hospitalisation
Compacks is a case managed discharge from Hospital that includes a package of Care for up to six weeks after leaving hospital. This Service seeks to ensure that people can return home safely and with appropriate care in place to heal well and to avoid unnecessary re admission.
The Home and Community Care Program (HACC) is a joint initiative of Federal and State Government providing a range of services to assist people who are frail aged and younger people with a disability, and their carers, one of which is Home Modification Services (HMS).
Kurri Contracting Service (KCS) is an Australian Disability Enterprise part funded by the Commonwealth Department of Family, Housing Community Services and Indigenous Affairs and auspiced by Kurri Kurri Community Centre Inc.
NSW Statewide Home Modification Service is auspiced by Kurri Kurri Community Centre providing major home modifications under the HACC (Home & Community Care) program, which is jointly funded by State & Federal Governments.
Kurri Kurri Community Bus Service is a transport service with 2 modified buses. Kurri Kurri Community Centre is responsible for the co-ordination of both buses in this transport service. Both buses are accessible to HACC eligible client groups and community groups.
